When is a voluntary health insurance contract terminated?
Voluntary health insurance agreement may be terminated:
- by way of a written notification of resignation to the National Health Fund Branch,
- after a month of continued arrears with the payment of premiums,
- upon changing the place of residence of the insured person outside the borders of Poland,
- as of the date of validation of the decision of the Branch director to determine proper legislation on the basis of the provisions concerning coordination confirming the competence of legislation of another member state of the EU or EFTA.
