What are the duties of the insured after signing the agreement?

Obligations of the insured person after signing the agreement:

- application for health insurance in Social Insurance (ZUS) on ZUS ZZA form ,

- payment of health insurance premiums to the 15th day of the next month for the previous month,

- revision of the amount of premiums in accordance with the changing basis.
